Camp Sunshine Individuals Application Form

Camp Sunshine Program Fees: $7 per day, $32.00 for 7 weeks or $64.00 for 14 weeks. The program fee in September is $5 per day. The program fee can be reduced or waived if a senior is unable to afford it.

Program fees can be paid by cash or cheque in person at Camp Sunshine, or sent by mail in advance to: Sunshine Centres for Seniors, 1033 Pape Ave., Suite 10, Toronto, ON M4W 2H2

1st Session – May 24- July 8:

WAIVER

I acknowledge that participation in or attendance at the programs, activities or events of Sunshine Centres For Seniors may involve certain risks due to the possibility of accidents and that I am solely responsible for my personal safety, well-being, and health while participating in or attending any such program, activity or event. I understand that photos taken during the program may be used by Sunshine Centres For Seniors for publicity or fundraising purposes only and hereby grant permission for their use.

In consideration of my being permitted to participate in or attend such programs, activities or events, I and my heirs, release and indemnify Sunshine Centres For Seniors, their directors, employees and volunteers against lawsuits, claims, or costs as a result of anything that may happen to me or my property as a result of my participation or attendance.
